Differential diagnostics.Date: 2015-10-07; view: 476. Medial caries must be differentiated with a deep caries and chronic periodontitis. When we differentiate the acute medial and deep caries , the depth of a carious cavity after preparation must be emphasized. Probing of a quite thin layer of the dentine of the carious cavity floor causes pain when at acute medial caries the walls of the carious cavity are more sensitive for the probing. At chronic periodontitis the tooth color changes. Preparation of the enamel-dentine junction is painless. Deep cariesis diagnosed in the permanent teeth quite often. This stage is characterized by the fact that there still stays a small layer of dentine before the pulp. The complaints: about dental pain caused by the temperature stimuli. The deep chronic caries is characterized by pain after cold, after removal of the stimulus it disappears at once. At deep development of a caries the pain appears after all kinds of stimuli ( cold, sweet, food particles). After elimination of the stimulus the pain disappears at once. Development of the disease: the pain developed a few weeks ago (at acute development), some months ago ( at chronic development). Objectively: at acute deep caries the color of impending enamel is like that of the chalk (at acute development the cavity is situated within the peripulpar dentine, it may be filled with the food particles). The floor probing may be painful (at acute development of the process).
